STUDENT ENROLLMENT AGREEMENT

ASSISTANT TRAINING PROGRAM

REGISTERED WITH UTAH NURSING ASSISTANT REGISTRY

COURSE OBJECTIVES:

The need for dependable bedside nursing care is as prevalent as ever, ranging from hospitals to home health agencies to long-term care facilities.  The Certified Nursing Assistant, or CNA, plays an important role, providing basic care while ensuring the safety and comfort of patients.   My CNA Source prepares the student for immediate employment as a nursing assistant.

Students are taught to recognize the needs of individualized patient care and to identify and select the appropriate skills and perform them in a safe and timely manner.  Students receive instructor-demonstrated skills and exhibit learning by return demonstration and supervised practice.

CURRICULUM:

The CNA training Program is an 82 clock hour program.  Classroom instruction includes an introduction to healthcare, basic nursing skills, body structure and function, infection control, and the job-seeking process.   Students also participate in supervised clinical activities.  Upon successful completion of the program, students are eligible to take the Utah state CNA certification examination.

CLASSROOM INSTRUCTION:

Our classroom is located in the Southwood Medical Plaza at 870 East 9400 South Suite 108 Sandy, UT.  Classroom entrances are in the front of the building and directly at the back.  Parking surrounds the building providing easy access to the classroom.  Each class is taught by a licensed nurses approved by the Utah Nursing Assistant Registry.  A textbook is provided for each student.  Each classroom is supplied with the equipment, such as hospital beds, wheelchairs, lifts and medical supplies to provide hands on training.  Audio and visual training aids are also used as part of the classroom instruction.  (56 hours).

CLINICAL ACTIVITIES:

Clinical hours are designed to allow the student to apply the skills learned in the classroom to an actual clinical setting.  Students demonstrate an understanding of nursing principles that ensure safe knowledgeable, skilled care and, under the supervision of the instructor, provide this care to assigned clients.  (16 hours)

ADMISSION:

All applicants must meet the following for admission to the CNA Training Program:

Applicants must be 16 years or older.  Those applicants under 18 and without a high school diploma, need to provide a parent or guardian signature.

Completed application with a $75.00 application fee.   The application fee will be applied to cost of the program and will hold the student’s seat.

ENROLLMENT:

My CNA Source welcomes applications from all qualified individuals.  Equal educational opportunities are offered to all who meet entrance requirements, without regard to race, gender, ethnicity, creed, marital status or age.

GRADUATION REQUIREMENTS:

Students must attend 100% or all scheduled classroom hours (56) and attend all clinical hours (16), and obtain a passing grade of 80% on all exams.  Any and all missed class hours or unsuccessful test scores must be made up prior to successful completion of this class.  Make up time will be made available through scheduling with the instructor.  After successful completion a Certificate of Completion will be awarded in addition to an application for the State Certification exam.

TUITION AND FEES:

At this time we do not offer any form of financial assistance.

Total course cost – $375.00

$75.00 —- Reservation fee to hold student’s spot in class. After the third day of payment, the fee will be non-refundable.

$300.00 —- Tuition.  Balance due by beginning of first day of class.

Course Fees Include:

  •   58 hours of classroom instruction
  •   16 hours of clinical instruction
  •   Student workbooks and Materials
  •   Certified Nursing Assistant Candidate Handbook for the State of Utah
  •   Practice tests

Course Fees Do Not Include:

  •  State Certification Fee
  •  Stethoscope and sphygmomanometer
  •  TB test
  •  Scrubs

Student Requirements:

  •  Students must come prepared to class with proof of having had a Tuberculosis (TB) test.
  •  Recommended to have a Blood cuff
  •  Recommended to have a stethoscope or sphygmomanometer
  •  Scrubs

 

CANCELLATION POLICY:

Should the student cancel or terminate for any reason, all refunds will be made according to the following policy:

  1.  Notice of cancellation must be made in writing and can be delivered by mail, email or in person.
  2. The $75 reservation fee is non-refundable after third day of payment
  3. Students may receive a full tuition ($300) for three days following the date the enrollment agreement is signed or from the date the student visits the institution, whichever is later.  Refunds will not be given after three days.  However, if a student needs to drop the class, tuition can and will be applied to another course to be taken within the next 6 months.
  4. A scheduled course may be rescheduled or cancelled if less than 2 students are registered.  In this event a full refund will be granted within three days of notification or the tuition can be applied to another scheduled class.

A student may be dismissed at the discretion of the Program Coordinator due to the following:

  1. Repeated tardiness and or class absences without prior notification to the instructor.
  2. Failure to make up excused or unexcused absences within 6 months from class beginning date.
  3. Disruptive behavior, foul language, possession of unperceived drugs or alcoholic beverages.
  4. Theft of property from the school, clinical setting, staff, other students or others.
  5. Failure to arrive a clinical location in appropriate uniform. One warning will be given before dismissal.
  6. Failure to pay tuition costs.
  7. Failure to adhere to clinical site policies and protocols.
  8. Any cheating  defined by the following:
    1. Communication between examinees inside of the examination room or facility during the course of the examination;
    2. Communication about the examination with anyone outside of the examination room or facility during the course of the examination;
    3. Copying another examinee’s answers or looking at another examinee’s answers while an examination is in progress;
    4. Permitting anyone to copy answers to the examination;
    5. Substitution by an applicant or by others for the benefit of an applicant as the examinee in place of the applicant;
    6. Use by an applicant of any written, audio, or video material or any other mechanism not specifically authorized during the examination for the purpose of assisting an examinee in the examination;
    7. Obtaining, using, buying, selling, possession of or having access to a copy of the examination prior to administration of the examination.
  9. Use of cell phones, itouches, ipods, mp3’s, ipads, and any other use of electronics in class is prohibited.

Students dismissed from the program may be eligible for readmission to another class during the next 6 months.  Readmission will be based on the severity of the infraction and at the discretion of the Program Coordinator.  Students with more than one dismissal will not be eligible for readmission.

SUCCESSFUL COMPLETION OF THIS NURSING ASSISTANT TRAINING PROGRAM WILL ALLOW YOU TO SIT FOR THE STATE OF UTAH ADMINISTERED COMPETENCY EXAMINATION IN ORDER TO BECOME A CERTIFIED NURSE ASSISTANT AS DEFINED BY STATE AND FEDERAL LAW.  MY CNA SOURCE MUST FORWARD IDENTIFYING INFORMATION TO THE STATE OF UTAH TO VERIFY YOUR ELIGIBILITY TO COMPLETE THE CERTIFICATION EXAMINATIONS.

THE STATE CERTIFICATION EXAM IS NOT INCLUDED IN THE PROGRAM NOR IS THE STATE EXAM FEE COVERED BY THE TUITION FEE.

STUDENTS ARE REQUIRED TO PRE-REGISTER AND MAY DO SO BEFORE THE START OF THE REQUESTED CLASS.

Upon successful completion of the program, a certificate will be issued to each student who successfully completes the program and satisfies all requirements.  My CNA Source Certificates only state that the student has completed the class and clinical requirements needed to proceed to the State Exam.  It is NOT an approved certificate for matter of employment as a CNA.
